PROVISION OF SERVICES SOCIAL WOKER F/T DAYS
Completes pre-discharge evaluations from patient/family interviews and healthcare team collaborations. Based on evaluation, develops a viable plan of intervention for current needs and potential discharge needs with consideration of illness/injury, living situation, support systems, and financial status.
PROVISION OF SERVICES
Demonstrates leadership in the provision of departmental services through a collaborative process designed to integrate services/functions throughout NWTHS. Assesses or coordinates the assessment of patient healthcare and education needs and collaborates with all members of the healthcare team to meet those needs. Orchestrates patient hospital process for provision of seamless, efficient care. Demonstrates the ability to effectively allocate human and material resources to meet patient care needs. Coordinates utilization management and insurance reviews as indicated. Complies with annual education requirements and safety and injury prevention standards to minimize risk to self and co-workers.

Qualifications
JOB TITLE: Social Worker (BA/BSW) (BA/BSW)
EDUCATION, TRAINING, EXPERIENCE AND LICENSES/REGISTRATIONS REQUIRED:
Bachelors degree in social work or in related field with a minimum of two years experience preferred as licensed Social Worker (BA/BSW)
Must be able to take the Social Work Examination within 60 days of hire.
Licensed by the Texas State Board of Social Work Examiners or is eligible and must obtain license within 6 months.
