Provides direct and indirect patient care under the direction of a registered nurse. Provides for the personal care and comfort of psychiatric patients. Provides a clean, safe environment, maintains equipment and supplies. Participates in performance improvement and continuous quality improvement (CQI) activities.
* Assures each new patient is oriented to the unit rules, patient bill of rights and responsibilities. Records and processes patients and their belongings on admission and at discharge
* Performs and records patient vital signs, intake and output, or other tasks as requested and notifies RN on any changes and charts them in the EHR
* Provides general nursing care, i.e., assisting patients with ADL, making beds, transferring patients. Performs patient care responsibilities considering needs specific to the standard of care for the age of the patient
* Continuously observes patient behavior, mental status and activities. Notifies RN of any changes or unusual occurrences. Performs observational rounds and suicide and seclusion checks per established policy, and tours unit with off-going shift to account for all patients
* Demonstrates the ability to lead and document rehab groups
* Demonstrates the ability to identify patient behaviors that require intervention. Continuously observes significant behaviors, actions and activities of the patients. Documents observations in the patients charts, reflecting implementation of the treatment plan. Reports any unusual occurrences
* Possesses the skills of verbal de-escalation and able to avoid power struggles with patients. Sets limits for patients. Ability to provide trauma informed care to all patients and families
* High School Graduate
* CPR/First Aid Certification
