Job summary
Case Managers provide case management which is a collaborative process with clients of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's needs through communication and resources to promote well-being. Essential Job Functions: Ability to communicate ideas in oral and written form. Ability to instruct in group and individual settings. Physical, mental and emotional health adequate to perform duties. Ability to work effectivity with cultural/ethnic diversity. Maintain confidential information according to legal and ethical standards. Ability to work at various locations, including client homes, schools and community settings. Effectively use de-escalation skills and techniques for crisis prevention. Properly administer the C-SSRS to help prevent suicide. Due to the State Public Health Officer Order of August 11, 2021: All workers in a public or private K-12 school facility are required to either be fully COVID-19 vaccinated with documentation to that effect on file or to undergo weekly diagnostic screening testing by October 15, 2021 to enter these school worksites. Responsibilities: Provide access, advocacy, resources and linkage to clients. Provide outreach services to include identifying areas that need to be strengthened, in addition to setting and reaching targeted goals. Interact productively within a multi-disciplinary team and other community organizations. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract. Attend community events. Report any suspected child abuse immediately to direct supervisor or utilize the chain of command if supervisor is not available. Report any suspected adult/elder abuse immediately to your direct supervisor or utilize the chain of command if supervisor is not available. Report any imminent danger to self or danger to others to direct supervisor or utilize the chain of command if supervisor is not available. Attend staff meetings and trainings. OTHER: Everyone who works in the organization has responsibilities for preventing and controlling infection. Everyone who works in the organization has responsibilities for maintaining a culture of safety. Other duties as assigned.
