Job Description: As a Registered Nurse with our hospice team, you will play a crucial role in providing comprehensive end-of-life care to patients in their homes, hospice facilities, or other care settings. You will work closely with a multidisciplinary team to ensure that patients receive high-quality, holistic care that addresses their physical, emotional, and spiritual needs.
Key Responsibilities:
Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical, emotional, and spiritual needs and collaborate with the interdisciplinary team to develop individualized care plans.
Medication Management: Administer medications and treatments as prescribed, ensuring effective pain and symptom management, while also educating patients and families about medication regimens.
Comfort and Support: Provide emotional and psychosocial support to patients and their families, helping them navigate the challenges of end-of-life care and offering compassionate guidance.
Symptom Management: Monitor patients for changes in symptoms and provide timely interventions to maintain their comfort and quality of life.
Education: Educate patients and families on end-of-life care, pain management, and disease progression, empowering them to make informed decisions.
Documentation: Maintain accurate and timely electronic medical records (EMRs), documenting patient assessments, care plans, interventions, and outcomes.
Collaboration: Collaborate with the hospice interdisciplinary team, including physicians, social workers, spiritual counselors, and other healthcare professionals, to provide holistic care.
Family Support: Offer guidance and support to families in coping with the emotional and practical aspects of end-of-life care.
Adherence to Regulations: Ensure compliance with all state and federal regulations, as well as hospice policies and procedures.
Qualifications:
Current and valid Registered Nurse (RN) license in state of practice.
Minimum of 5 years of nursing experience, preferably in hospice or palliative care.
Excellent communication and interpersonal skills.
Compassionate and empathetic approach to patient care.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in electronic medical records (EMR) systems.
BLS (Basic Life Support) certification.
Strong critical thinking and problem-solving abilities.
Reliable transportation and a valid driver's license.
